SENSORY TAEKWON-DO

1. LIABILITY AGREEMENT

Martial arts, tumbling, cardio and other physical fitness activities have inherent risks. Injury may occur to participants or guests. We use caution while training children however accidents may be

unavoidable at times. Because of the inherent risks with physical activity, it is required that children are always attended to and brought into the training area with a Sensory Taekwon-do instructor. Knowing these risks, parents or the guardian/aide of a student/member must understand and adhere to instructions and participation as explained here. The instructor(s) may need to hold the student/member by hand, arm, leg or body to achieve stimulation and therapy goals and at other

times to avoid injury or harm to student and/or instructor. The parent, guardian/aide understands that there is a minor risk of injury during the activities and instruction and indemnifies and save harmless the center/facility, its owners, instructors, officers and all other related members of the center/facility from all loss caused by accident or injury to the student/member or any persons accompanying student/member.

2. MULTI-MEDIA CONSENT

I hereby give Sensory Taekwon-do permission to use pictures and videos of my child to promote the expansion of the program on social media. The child/students' pictures may appear in flyers, brochures and other multimedia formats. I understand that the images may be modified to be used as design elements.

I have read this agreement and fully understand the contents herein.

3. MAKE-UP POLICY

To maintain the highest standard of service we will need to reinforce

our current make-up policy.

Your membership guarantees:

● a discounted rate

● a specially trained instructor

● your reserved appointment time

For us to provide quality training and have consistency for your child

the current policy is as follows:

● NO show, NO call your class is forfeited

● 8 hour notice for cancellations qualifies for a make-up. If 8 hours

are not given your make up class is forfeited.

● Sick make-up with less than 8 hour there is a $15.00 fee

● One make up attempt is allowed

● Cancellations can be made via text, email, or office call
